Team Effectiveness
How functional is your team today?
A self-assessment against the five dysfunctions model. Twenty-five statements, about five minutes.
Before you start
Rate each statement as you experience your team right now, not as you would like it to be. Answer quickly and instinctively. First reactions are more honest than considered ones, and over-thinking flattens the result.
Your scores are grouped and interpreted once you finish. The groupings are deliberately not shown beforehand.
- 1 Never
- 2 Rarely
- 3 Sometimes
- 4 Usually
- 5 Always
Your results
The layers run in order from the foundation upwards, so Trust is first. The model is cumulative, and a weak foundation drags down every layer that follows it. A low Results score caused by low Trust will not shift until Trust shifts.

Score detail
| Layer | Total | Average | Range | Band |
|---|
Bands: High is 3.75 and above, Medium is 3.25 to 3.74, Low is 3.24 and below. Because each average is a total out of 25 divided by five, scores move in steps of 0.2, so in practice Low is 3.2 or below and High is 3.8 or above. The midpoint of the scale is 3.0, which sits in Low. The bar is deliberately demanding. Range is the lowest and highest answer you gave within that layer. A wide range under a comfortable average usually matters more than the average itself.
